Voorhees Pediatric Dentistry Membership Plans

At Voorhees Pediatric Dentistry, we want to give our patients without dental insurance a plan that could offer them similar savings, without the hassles of deductibles and claim forms.

When you join the Voorhees Pediatric Dentistry Membership Plan, you will receive:

• One Comprehensive Exam (1st year only)
• Two adult or child cleanings with fluoride per year (MUST BE USED WITHIN THE 12 MONTH PERIOD OF CURRENT CONTRACT)
• One healthy periodic dental exam for the first year and then two per year after first year
• One emergency exam and any individual x-rays
• Two Oral Cancer screenings per year
• One set of x-rays (typically 2 or 4 bitewings and 2 Periapical x-rays), only for “Child and Teen Plan”
• Panoramic x ray or Full Mouth Series of X-rays (1 every 5 years), only for “Child and Teen Plan”
• 15% off other preventive and restorative procedures completed at the office, such as sealants, fillings, crowns, extractions, or any extra exams, cleanings, or fluoride

Joining the Dental Membership Plan could not be easier and more affordable. For just $399 per person per year for the “Infant Dental Care Plan,” and $450 per person per year for the “Child and Teen Care Plan” you can enjoy savings on dental services. With the Dental Membership Plan there are no complicated forms to fill out, or cards that you need to remember to bring.

The “Infant Dental Care Program” is for ages 0-2 (lasts until the end of the last month of their second year, and the “Child and Teen Care Plan” is for ages 3 through graduation from practice)

The Dental Membership Plan is NOT dental insurance and CANNOT be used in conjunction with ANY dental insurance plan or with ANY other discounts, including third-party financing such as Care Credit. There are no refunds with the plan. The plan runs for one year from the date you sign up with full payment. These benefits are ONLY for the plan period. This plan cannot be used for costs of dental care which are covered under automobile or medical insurance, for referrals to specialists and for treatment which, in the sole opinion of our doctor, lies outside the realm of their capability. These plans DO NOT cover procedures completed by doctors or providers outside the practice and/or services provided by an independently contracted dental anesthesiologist. Discounts ARE NOT transferable to outside doctors, providers, or specialists or any independently contracted providers or specialists within the practice

Both plans are in-office discount plans arranged between the patient and Voorhees Pediatric Dentistry for services provided within our practice.

These membership plans ARE NOT dental insurance plans or qualified health plans under the Affordable Care Act and cannot be combined with any dental insurance plans.

All family members must live in the same household. If children of the same household do not share the same family name, legal documents may be requested for identification and/or proof of custody.

Membership Plan dues must be PAID IN FULL before enrollment begins.

The effective day one of your plan is the day of enrollment and expires on the 365th day of your plan year.

If payment for any additional exams, x-rays, or procedures is required, all monies must be paid in full at the time of service.

All covered procedures must be completed during your coverage year. Unused covered procedures WILL NOT be rolled into the following year.

Membership fees or the percentage of discount is subject to change on a yearly basis.

Procedure fees are subject to change at the office’s discretion (office fees are assessed and adjusted annually on January 1 of each year)
